Your Support at Home provider arranges the device and claims it from your AT-HM funding, and a participant contribution can apply. Ask your provider whether the KISA Care monitoring plan can also be included.
Only if you want the device funded. AT-HM funding is approved through an aged care assessment and set out in your Notice of Decision and support plan. If you moved across from a Home Care Package, you can use the AT-HM scheme without a new assessment, and any unspent Home Care Package funds are used first.
